The Wall of Honor application
The Wall of Honor application is to be filled out by a member of the honoree’s organization with assistance from the Missouri EMS Funeral Response team or the Missouri EMS Memorial Foundation. The honoree’s name, organization, and date of last call/mission will be placed on the Wall of Honor in remembrance of his/her ultimate sacrifice.
The Service Wall application
The Service Wall recognizes the service of those who have faithfully dedicated their lives to Missouri EMS. Those names may be etched in to one of the service walls which flank the Wall of Honor. Thus, watching over our fallen EMS brothers and sisters.
To qualify for inclusion on a Memorial Service Wall, the following criteria should be met:
- An individual who has served a minimum of ten years in the EMS profession with the conclusion of their service in a State of Missouri Agency in good standing.
- EMS Professionals not meeting the 10-year requirement, under extraordinary circumstances, may be considered on a case-by-case basis at the discretion of the current Board of Directors. For this to occur the application should be completed and include a letter stating as much detail as possible about the individual’s circumstances and a minimum of three letters of recommendation. These applications do not guarantee inclusion on the Missouri EMS Memorial.
- Individuals who have shown superior dedication to the EMS Profession in the State of Missouri may be considered on a case-by-case basis with approval of the current Board of Directors. This may include, but not limited to, individuals who have served as a local/state advisor, medical director, or educator. For this to occur the application should be completed and with it a minimum of three letters of recommendation submitted. These applications do not guarantee inclusion on the Missouri EMS Memorial.
